VITAMIN A DEFICIENCY

This 64YO man presented with 1 year of gradual severe bilateral vision loss. Vision was counting fingers at 5 feet OU. Retinal examinations showed fine subretinal pigmentary changes with tiny whitish dots throughout the extramacular retina, best visualized as hypoautofluorescent spots on Optos ultra-widefield autofluorescence. Triton swept-source OCT shows profound loss of the outer segment… Read More

VITREOMACULAR TRACTION + LAMELLAR MACULAR HOLE

This 70YO female presented with 20/200 vision in her right eye from severe vitreomacular traction with a tractional lamellar macular hole (LMH). OCT scanning shows a highly elevated posterior hyaloid adherent to the underlying fovea, with tractional splitting of the outer plexiform layer (OPL). There is also a faint nasal epiretinal membrane. Combined cataract surgery,… Read More

BEAR TRACKS

This 44YO presented with these asymptomatic pigmented lesions in his left eye. He is healthy and has no family history of colon cancer. These lesions most likely represent an atypical case of group-type congenital pigmented nevi of the RPE (bear tracks). This is a relatively rare congenital condition characterized by well-demarcated, hyperpigmented, flat, variably sized… Read More

SEPHIROTH RETINOPATHY

We all know it’s a large subhyaloid hemorrhage from proliferative diabetic retinopathy, but check your imagination and tell us what you see. We see Sephiroth from Final Fantasy!

VALSALVA RETINOPATHY

This 56YO female presented with acute vision loss in her left eye from Valsalva retinopathy. Vision was 20/100. A temporal foveal retinal hemorrhage is seen, located within the inner retina on OCT B-scan. Five weeks later, the devitalized blood is contracting and turning yellow. The blood also became more hyperreflective on the OCT B-scan. All… Read More

PROLIFERATIVE DIABETIC RETINOPATHY (PDR)

This 41YO female presented with bilateral proliferative diabetic retinopathy (PDR). Optos ultra-widefield color imaging shows extensive bilateral retinal hemorrhages, along with hard exudates and cotton wool spots. Fluorescein angiogram shows extensive scattered retinal neovascularization with more peripheral retinal ischemia.

NEOVASCULAR AMD

This 77YO male presented with counting fingers vision OD from a large fibrotic disciform scar and 20/200 vision OS from a new macular neovascularization. The disciform scar is contracted and fibrotic, with marked hyperreflectivity on swept-source OCT. The MNV in his left eye is seen as opaque fluid with some subretinal blood. The type 2… Read More

PERIPHERAL EXUDATIVE HEMORRHAGIC CHORIORETINOPATHY

This 94YO female presented with ectopic choroidal neovascularization (CNV), also known as peripheral exudative hemorrhagic chorioretinopathy (PEHCR) in her right temporal periphery. Inferior to this lesion are clumps of subretinal pigment. Confluent subretinal lipid is also noted more inferiorly. Vision was 20/400 due to macular edema from an unrelated major superotemporal branch retinal vein occlusion… Read More

PAPILLEDEMA

This 22YO female presented with severe bilateral optic nerve edema and enlarged blind spots. She reported recent weight gain following the placement of an IUD. MRI and MRV scanning were normal. Lumbar puncture opening pressure was extremely elevated at 55cm H2O, establishing the diagnosis of idiopathic intracranial hypertension (pseudotumor cerebri). She was evaluated by neurosurgery,… Read More

CUTICULAR DRUSEN

This 62YO male presented with 20/20 vision OU and asymptomatic macular drusen. Optos photos of both eyes show numerous scattered cuticular drusen throughout each macula. Fluorescein angiography shows a dramatic bilateral starry sky pattern of innumerable hyperfluorescent drusen. Triton swept-source OCT reveals multiple tooth-like cuticular drusen. Learning Points: Cuticular drusen, formerly known as basal laminar… Read More
